The Community & School Collaborative (CSC) started as joint project of Guadalupe Alternative Programs (GAP) and Change Inc. to remove mental and emotional barriers to school success. Today, CSC has mental health practitioners and professionals in 28 Twin City metro schools to provide on-site school-based mental health services to more than 650 children and families.
School-Based & Outpatient Mental Health Services
Our School-Based & Outpatient Mental Health Services are provided at school during school hours, in Out-of-School Time in the community, and in the home. Schools, school districts, and government agencies all contribute to funding these efforts (in addition to insurance reimbursements). With this service model, we also provide staff development to equip teachers and other staff members with methods to be more trauma-informed in their work with students. We also work with parents and support their efforts to provide nurturing home environments.

Mental Health Support Line
Change, Inc. has opened a Mental Health Support Line that is staffed by mental health professionals and practitioners. Individuals, families or school staff may call this number for brief phone support, referrals to community resources, or ongoing mental health services. They have the capacity to provide phone support in Burmese, English, Hmong, Karen, Poe Karen, Spanish, and Thai.
